RISK – Lift Failure
Refer to the Lifting Operations Competency Matrix & the Lifting Operations Procedure for lifting competencies.
Anyone undertaking high risk work must have the relevant high risk work licence and be verified as competent in accordance with the Verification of Competency (VOC) Procedure (linked below).
Lift Plans must be developed, checked and authorised in accordance with the Lifting Planning Matrix.
Any changes to lifting operations must trigger a review of the Lift Plan by the Lift Planner to determine whether the lift requires re-categorisation.
Use the Lift Planning Matrix to determine whether the lift is a critical, medium or low risk lift. Factors such as the weight of the load, multiple hoists/load share, rigging requirements, lifting configuration and ground conditions will help determine the risk level.

Mandatory competencies for a Lift Designer:
- Workplace Induction
- Lifting Operation Procedure familiarisation training
- Lift Designer Training
- Lift Designer – Level 2 – Competency Assessment (Theory)
- Mentoring with a Heavy Lift Specialist
- Demonstrated experience in the lift design process (Practical)
- Workplace Manager Approval
- Competency letter issued
Mandatory competencies for a Heavy Lift Specialist:
- Workplace Induction
- Lifting Operation Procedure familiarisation training
- Lift Designer Training
- Lift Designer – Level 2 – Competency Assessment (Theory)
- Experience in the lift design process, use of crane specialist software and AutoCAD
- Minimum 8 years previous relevant crane, rigging & transport experience
- Regional General Manager Approval
- Competency letter issued
A Verification of Competency is a method used to determine the ability of an individual (both JH and subcontractors) to competently perform high risk work or operate an item of plant or equipment safely. This John Holland requirement is in addition to the attainment of any high risk work licence and/or other qualifications.
