RISK – Contacting live services
The risk assessment must include: competency requirements for workers, a detailed isolation method, labelled and lockable isolation points, confirming a “zero energy” state, verifying isolation details in the field, locks and tags firmly secured to the isolation point.
Special isolation requirements should be identified and planned in the Workplace Risk Assessment (WRA) and Activity Method Statement (AMS). Examples of special isolation requirements include hazardous chemical sites, offshore working or other unusual hazards.
These requirements should be addressed by SMEs and HSE personnel with site specific processes implemented.
An isolation may require other types of permits to be raised as part of the scope of work. This may include:
- Confined space entry permit
- Working at height permit
- HV access permits – for access to high voltage equipment
- Excavation and trenching permit
- Hot works permit
- Core hole permit
Isolation inspections and high-risk audits will be conducted by the project team in accordance with Workplace Hazard Identification and Inspection Procedure and Monitoring and Review Procedure to ensure isolation controls are monitored on site.
