RISK – Untrained workers performing an isolation
Workers carrying out isolations/de-isolations must have relevant qualifications within the isolation discipline.
Any person involved in the isolation must complete the Isolation procedural awareness Training.
Once the work is complete, the work area must be systematically de-isolated to reinstate plant or equipment integrity. The Permit Holder or Isolation Officer must inspect the area to ensure that:
- Non-essential items have been removed and that equipment components are intact
- All personnel are at a safe distance from the area where energy is to be returned to the equipment
- All guards and safe systems have been reinstated
- Confirm that all workers have removed their personal locks and the work has been completed
- Check to ensure all permits issued for the work have been signed off and returned to the Permit Controller to acknowledge completion.
- The Permit Controller will then close the permit
Specialised training for high risk operations (e.g. HV switching) must be provided and the operator verified as competent.
Apprentices cannot work under an isolation by themselves; they must always be supervised. Apprentices cannot act as Permit Holders.
