RISK – De-isolating without necessary controls in place
The Permit Holder or Isolation Officer will inspect the area to ensure all guards and safety systems have been reinstated, all locks have been removed and all associated permits have been signed off.
Once the work is complete, the work area must be systematically de-isolated to reinstate plant or equipment integrity.
Prior to de-isolation, the Permit Holder or Isolation Officer must ensure:
- Non-essential items have been removed and that equipment components are intact
- All personnel are at a safe distance from the area where energy is to be returned to the equipment
- All guards and safe systems have been reinstated
- Confirm that all members of the work crew have removed their personal locks and the work has been completed
- Check to ensure all permits issued for the work (if required) have been signed off, and returned to the Permit Controller to acknowledge completion
