RISK – Removing isolation tag/lock prematurely
The Supervisor will complete the Authority to Remove Personal Danger Lock Form if required.
The red Personal Danger Lock and Tag are only to be attached and removed by the person whose name appears on the tag.
Prior to de-isolating the isolation point, the Permit Holder, or Isolation Officer for complex sites, must confirm that all tags and locks have been removed. Where tags/locks remain, this must be immediately reported to the Permit Holder. De-isolation cannot continue until all remaining tags/locks have been removed.
